GENERAL INFORMATION:

Only a parent or legal guardian may register a student.

Parents who complete registration during the special kindergarten sessions in March will be advised of their child's assignment by July 1 of that year.

BIRTH CERTIFICATE:

A child's original birth certificate/passport must be presented at time of registration. A copy will be made for the school file. In order to be eligible for kindergarten, a child must be five years old on or before October 15 of this year. There are no exceptions to this Board of Education policy.

PROOF OF RESIDENCY:

All families must complete the Statement of Parent/Guardian Affirming Residency in the Borough of North Plainfield, NJ (NP REG. 0001a-0001b). Although this form does not have to be notarized, it does require the signatures of husband and wife if parent is married.

Two Original documents must also be presented as proofs of residency. One of the following documents must be submitted from each of the two categories as proof:

Category 1

1. Current Lease
or
2. House Deed/Tax Bill   (Mortgage commitment papers are not accepted)

Category 2

1. Current Utility Bill (Gas, electric, phone)
or
2. Pay Check with new address shown
or
3. Current Bill (Hospital, doctor, credit card)

If parents are living with a relative/friend who is renting an apartment, the names of both families must be on the lease. If no lease exists, the landlord must complete the Affidavit of Landlord/Owner Resident of North Plainfield, NJ (NP REG. 0005a, 0005b, 0005c). This form must be notarized and returned.  A current utility bill or pay check with the name and address of the student's parent is also required.

Similarly, if parents are living with a relative/friend who owns a house, a bill or pay check with the name and address of the student's parent is also required. The owner must also show proof of residency. The Affidavit of Landlord/Owner Resident of North Plainfield, NJ (NP REG. 0005a, 0005b, 0005c) must also be completed and notarized along with a copy of the landlord/owner's tax bill or deed.

IMMUNIZATIONS:

A child's immunization records must be up to date. The records must show the month and year of each inoculation.  

PARENT/GUARDIAN IDENTIFICATION:

Parent/Guardian must show identification with photo.
